 Cascade Medical is pleased to offer a secure Internet-based system that is designed so that you may receive certain confidential medical information about yourself via a Web-based version of our computer system called MyHealth.

 If you are visiting this page, then you take your privacy as seriously as we do. We think it is important for you to understand how we handle the information we receive from you and communicate via the Internet. This Terms and Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

Acceptance of Terms

 By agreeing to the following Terms and Conditions of Use, I acknowledge that I am requesting access to a portion of my personal health information and the ability to communicate with my Cascade Medical healthcare team regarding my personal health information via the Internet using the MyHealth portal. I hereby expressly authorize Cascade Medical to disclose my identifying personal health information to my designated MyHealth account.

Disclaimer on Emergency or Urgent Medical Conditions

 MyHealth should never be used in emergency situations requiring immediate attention. In an emergency situation, do not submit messages electronically. Dial 911.

Response to Electronic Communication

  Cascade Medical will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff may not be immediately available. Please allow at least three (3) business days for a response. Delays may occur based upon message volume, availability of staff, the complexity of the patient’s question, or technology disruptions. Therefore, Cascade Medical cannot guarantee that messages will be responded to within three (3) business days.

Web Site Links

  All health-related information and medical Web site links offered through MyHealth are intended for informational and educational use only and are not managed by Cascade Medical. Cascade Medical does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

Minor Eligibility

 An individual must be age 13 or older to request a personal account. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Access to Others’ Charts" section of this Terms and Conditions statement.

 Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 13 or older may not view without consent of the minor patient. Because of these requirements and the fact that the portal cannot shield certain portions of the records the following requirements are in place:

Parent or a legal guardian of a minor is eligible for access to the minor’s account until the child reaches the age of 13. If the patient is under the age of 13, access may be issued to the parent or legal guardian as long as no restrictions are issued in the form of an order by the court of law prohibiting them from acting as proxy for the child’s MyHealth information.

At the age of 13, the system will automatically unlink the minor’s chart from the parent’s MyHealth account.

To restore a disabled link to the child’s account or to establish a new link to the chart of the minor over the age of 13, the minor must complete a Release of Information form in person. This form acknowledges the patient’s approval of their parent or legal guardian’s access to all medical records available in the minors MyHealth account.

A minor 13 years of age and older may request that the parent or legal guardian be denied access to their medical records available through the MyHealth. This request will be honored. The minor age 13 years or older may create their own MyHealth account.

A Release of Information must be completed by the minor in person to authorize reinstatement of access to the parent or guardian.

Access to Others’ Medical Information

 Individuals age 13 or older can grant access to their individual MyHealth account to a proxy (another individual age 13 and older) by completing the Release of Information Form in person. MyHealth accounts will be activated for both the proxy and the individual, and messages and notifications will be directed to both the patient and the proxy.

If the legal relationship between the account holder and the patient changes, the account holder must inform the MyHealth Portal Support immediately.

Cascade Medical reserves the right to revoke proxy access at any time.

E-mail Privacy

 MyHealth users will be notified via e-mail when new medical information is available. This means that any person with access to a patient’s e-mail will be able to see this notification. Although no private medical information will be sent, patients should take this into account when providing an e-mail address.

 Please know that if you send us an e-mail communication, it may be shared with the Cascade Medical staff that assist physicians in providing medical care. A patient’s confidential medical information displayed in your MyHealth account will be accessible only to appropriate clinical staff.

Security and Confidentiality

 We afford the same degree of confidentiality to medical information stored in MyHealth as is given to medical information stored by Cascade Medical in any other medium. Cascade Medical is committed to protecting the confidentiality of your medical information. We limit Cascade Medical employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyHealth. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

 You can tell when you are secure by looking at the location (URL) field. If the URL begins with "https://" (instead of "http://"), the document comes from a secure server. This means your data is encrypted and cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Cascade Medical by viewing the digital certificate. This certificate verifies the connection between the Cascade Medical server's public key and the server's identification.

 User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyHealth, maintain the security of your medical information changing your password on a regular basis and keeping your login ID and password confidential.

Our company and its affiliates, suppliers, and other third parties mentioned on this are neither responsible nor liable for any direct, indirect, incidental, consequential, special, exemplary, punitive, or other damages (including, without limitation, those resulting from lost profits, lost data or business interruption)arising out of or relating in any way to this site, site-related services and products, content or information contained within the "site" and/or hyperlinked website, whether based on warranty, contract or tort or any other legal theory and whether or not advised of the possibility of such damages. Your sole remedy for dissatisfaction with the site, site-related services, and/or hyperlinked websites is to stop using the site and/or those services. Applicable law may not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to you.
